The CHCAMS will host a Virtual Fall Clinical Conference for the community health centers’ multi-disciplinary care teams providing care to patients of all ages and special populations. In so doing they are they are challenged with finding innovative ways to enhance population-based primary care and utilize patient information for care management that meets the needs of the patient. CHCAMS has a mission to provide the TA/training needed to assist in this process. Conference sessions will address optimization of the electronic health record to track and monitor population-based clinical markers and models of care management to improve patient outcomes.
